What is a Chronic Condition Management Plan and are you eligible for one?
Did you know you may be eligible to receive Medicare rebates on your osteopathy appointments?
If you’re living with a long-term health condition, you might qualify for a GP Chronic Condition Management Plan (CCMP), formerly known as an EPC plan. This plan can help you access allied health services (including osteopathy) with Medicare rebates to reduce your out-of-pocket costs.
What is a Chronic Condition Management Plan?
A CCMP is organised by your GP to support patients with chronic or complex medical conditions. Under this plan, eligible patients can access up to 5 allied health visits per calendar year, which may include osteopathy, psychology, physio, podiatry and other services. A CCMP provides a rebate of $61.80, which will go up again in July 2026.
Who is Eligible?
Only a GP can determine your eligibility.
Generally, you may qualify if you have a chronic medical condition that has been present (or is expected to be present) for 6 months or longer. Examples can include:
Arthritis or ongoing joint pain
Chronic back or neck pain
Osteoporosis
Diabetes
Cardiovascular disease
Autoimmune conditions

What is Required to get a CCMP?
To access a CCMP, you will need to:
Book a consultation with your GP to discuss.
Your GP will assess your medical history and determine whether you meet the eligibility criteria.
If eligible, your GP can prepare a GP Management Plan and Team Care Arrangements, and provide a referral to an allied health provider (such as your osteopath).
How Does It Work?
Once you receive your referral:
Bring your CCMP referral to your osteopathy appointment.
You will pay for your consultation as usual.
We process your Medicare rebate which will then be paid back to you by Medicare, usually the next business day.
Your Osteopath will write letters to your GP at the first and last sessions to update them on your progress.
You can claim rebates of $61.80 for up to 5 eligible sessions per calendar year, as determined by your GP.
